• Names and contact information of the responsible parties
• Any witness contact information
• Any police or incident reports
• Your automobile insurance information if your case involves an auto accident. (Specifically we need the Declaration Page from your insurance policy.)
• An estimate of repair to your vehicle
• Any photographs of damage to your vehicle
• Any photographs of the scene of the accident
• Photographs of injuries that occurred as a result of the accident
• The names and addresses of any medical providers that you have seen since the accident
